Waukegan Park District Travel Forms

Which form do I need?

FAQ

How is my per diem figured?

It uses the U.S. General Services Administration (GSA) daily rate for where you're going, times the number of full days. Meals included in the conference cost are subtracted.

Why do I have to enter included meals?

Conference-provided meals are deducted from the per diem. Enter how many breakfasts, lunches, and dinners are included. If none are, enter 0 on each line. The form won't print until all three are filled in.

What do I do with the Advance when I'm done?

Save it as a PDF and attach it, along with any conference documentation, to your BS&A Check Request.

What do I do with the Expense form when I get back?

Save it as a PDF (or use Download PDF to sign it in Adobe), get it approved, with the signatures at the bottom of the form, through your department's approval process, and attach it with your receipts to your BS&A Invoice. (Your Check Request becomes an Invoice in BS&A.)

I got a per diem advance. Do I still fill out the Expense form?

Yes. Mark "Was the Per Diem Received?" as Yes so the per diem isn't paid twice.

How do I claim mileage?

Enter your total miles in the Mileage section of the Expense form. The form fills in the district's current IRS mileage rate, and the reimbursement is calculated for you.

Why won't the form print?

Something needs fixing first, such as a start date after the end date or a blank meals line. A red message on the form says what to fix.

What policy covers travel?

District Policy 3.09, Travel, Meetings and Vehicle Use.
